Our South Liverpool Community Connect Fund, a partnership between Capacity and LCVS, helps voluntary sector organisations working in, or connected to, South Liverpool to support GP practices to connect their patients to community assets.
There are an increasing number of people presenting to their GP with non medical needs. This is leaving patients and GPs feeling frustrated. We know there are a wealth of community assets sitting across South Liverpool, however patients are not connecting with them as well as they could.
We are looking to commission a lead agency in South Liverpool that can help develop a local partnership with other small to medium voluntary, community and faith (VCF) organisations in the area to help design and test a solution to this problem. There is £25k available and it is expected that this will be used to help fund local activity that connects patients with local VCF organisations.
The successful applicant will work across primary care settings in the south of Liverpool to engage residents with community activity, working in partnership with other small to medium VCF organisations.
The maximum amount an organisation can apply for is £25,000.
The successful applicant should:
- Have strong existing relationships and work in collaboration with a range of VCS’s in the area
- Be driven by asset based community development, taking a strengths based approach to improving outcomes
- Show enthusiasm to engage in the service design and testing process
Closing date for applications: Monday 8th April at 5.00pm.
Funds must be spent and delivery completed by October 2019.
