Contract: 35 hours – full-time – fixed term until 31 March 2025 initially with a possible extension dependent on funding.
Term: Temporary.
Salary: £26,250 per annum.
Hours per week: 35 hours.
Location: The Frances Suite, Floor 1, 151 Dale Street, Liverpool, L2 2JH, City Centre office with regular travel across Sefton and Liverpool, flexible home working.

With nearly 100 years’ experience working with older adults in Liverpool & Sefton, they are experts in understanding the needs of their community and delivering impactful services.
Their services include: befriending and re-ablement, a Mental Health Support Service, friendship services, an Activity Programme operating from their activity room and at locations across the city, an information and advice service, a telephone, online and drop-in service for any query or concern raised by an older person, relative, neighbour, professional or other VCSE partner and The Hamlets, their 30-bedroom specialist nursing home for older adults with functional mental health needs.
Job Summary
Combat Isolation is a bespoke service based across Liverpool, Sefton and Kirkby. The team supports older adults (over 50) diagnosed with functional mental illness who have been affected by loneliness and isolation exacerbated by the impact of Covid. The service has been designed to build confidence and increase independence through delivery of one-to-one support. This will include developing a personalised community support programme and agreeing personal objectives with the client and a strategy to achieve them, encouraging clients to access meaningful activities to benefit their mental health, wellbeing and independence.
In the role, you’ll manage a client caseload delivering one-to-one support over 12 weeks, with a focus on either the Sefton or Liverpool area. Your focus is on encouraging social engagement, building networks, and facilitating access to community-based provision in line with client’s individual goals and aspirations. You will also support facilitation of drop-in sessions where clients can come together and make connections with each other, building a supportive community and providing social activities and signposting.
