Advice on Prescription Plus (APP) Social Prescribing Coordinator

Context of role

The Liverpool Advice on Prescription (AoP) was commissioned by Liverpool Clinical Commissioning Group (LCCG) in 2016 to improve preventative health outcomes for people with long-term conditions and/or co-morbid mental health problems. The service takes direct referrals from GP teams and other health professionals and is able to respond to urgent need as well as providing ongoing support. We are looking for an experienced programme coordinator to build and develop a highly motivated team who will work closely with health professionals and community organisations to support referred patients to make lifestyle changes.

The Liverpool Advice on Prescription Plus (AoP+) will enhance the existing service by blending in a holistic and more diverse health and well-being offer. The Social Prescribing Link Workers will provide supported access to well-being activities such as: exercise, connectivity, creativity and pathways to volunteering, employment, enterprise, education and learning.

The successful candidates will be based in our Garston office and will work within a busy and challenging environment to ensure that people referred by GP teams and other health professionals are supported and provided with meaningful links to well-being services. The programmes’ Link Workers will meet patients in a variety of settings and support them to develop a pathway to improved health and well-being. To provide this support you will need to develop a sound knowledge of local health, social and voluntary services
