Mersey Care Foundation Trust wants to find local partners from the voluntary and community sector to work with.
In April this year, Mersey Care Foundation Trust took on the provision of community health services for Liverpool and has been a lead on the establishment of a multi agency Liverpool Provider Alliance to design and implement a new community model of care which will use a personalised approach to optimise health and well-being outcomes.
The Provider Alliance vision is to work together as one team to provide the best possible services for our communities. From cradle to grave, Mersey Care will look to improve health and well being and promote greater independence and outcomes for people with people in control.
Mersey Care wants to work with voluntary and community sector partners to create a network that will ensure community assets are maximised and targeted on building resilience in our local communities. This will encompass the wider influences on health and well-being such as social, cultural, environmental and economic factors.
