Contract type: 2 years fixed term contract with possible extension, subject to funding.
Employment type: Full time.
Hours: 35 hours per week, between 8 am – 8 pm and may include weekends (rota basis).
Salary: £25,000 per annum plus pension.
Location: Speke, Woolton, Allerton, Gateacre, Garston and Aigburth Primary Care Network (SWAGGA PCN) – South Liverpool.
Responsible to: Health Junction & PCN Leads.
Accountable to: PCN Clinical Directors.
- As a Health and Wellbeing Coach, you will be a key member of the Multi-Disciplinary Team (MDT), working across the Primary Care Network (PCN) practices. You will work with individual patients and groups of patients holistically, considering the whole person. The role will have a focus on winter pressures, high users and those with multiple and long-term health conditions.
- Health coaching revolves around using coaching skills to support people with lower levels of patient activation to develop the knowledge, skills, and confidence to manage their health and wellbeing. The role also covers support for community safety, individual patient case management, follow up and ongoing patient support.
- The post holder will have a key role in helping to raise the local population’s awareness of the support groups and opportunities available to assist them in achieving their health and wellbeing goals.
