Job Summary
They are seeking individuals with experience in Health & Social Care, working collaboratively with multi-disciplinary teams, processing referrals, conducting assessments, and delivering person-centred services, with a commitment to reducing reoffending and improving psychological wellbeing. The role involves ensuring smooth referrals, applying formulation-based approaches, and managing risk and support needs. You will lead and support staff, build strong partnerships with statutory and third-sector services, enhance access to community resources, and work closely with housing providers and commissioners to ensure effective service delivery aligned with OPD Pathway goals.
