Job Summary

As a Resettlement & Recovery Support Worker, you will develop therapeutic relationships with individuals to support them through a range of emotional states and challenges. Working closely with colleagues, professionals, and key partners, you will create and review person-centred support plans that empower individuals to take an active role in shaping their care. You will encourage and motivate people to access socially inclusive opportunities, including employment, education, volunteering, sports, leisure, cultural, and creative activities. The role also involves supporting individuals to access NHS services, specialist employment programmes, welfare and legal advice, and to manage their accommodation and daily responsibilities. A central focus will be promoting the Five Ways to Wellbeing, helping individuals register with NHS services, and ensuring consistent support throughout their transition to independent living.

In this role, you will also undertake risk assessments, formulate comprehensive risk management plans, and recognise early signs of deteriorating mental health to help prevent relapse or re-offending. You will deliver and support group activities, work with landlords, Imagine’s housing department, and care co-ordinators to help individuals secure and sustain accommodation and liaise with statutory agencies to ensure joined-up support. Building positive relationships is essential, encouraging individuals to respect and consider the welfare of others in shared services. Alongside this, you will maintain accurate records for monitoring and evaluation, contribute as an effective team member, and ensure that any concerns regarding individuals are reported promptly to a manager.
