Description: LiveWire Liverpool are seeking to appointment a Social Prescriber Link Worker.
The role is for a fixed term period of 12 months with the possibility of an extension.
We are looking for an energetic individual who has the ability to engage with a wide range of individuals, supporting them to embark on lifestyle changes that make a difference to their wellbeing. The successful candidate will work in partnership with GP practices, patients and other partners across central Liverpool.
Actively engaging with the local community, you will be required to have extensive knowledge of facilities and activities within the local area as referral options for patients.
Identifying any gaps in delivery, you will be expected to support local community groups in accessing funding opportunities to establish new groups.
