Living Well Sefton – Senior Social Prescribing Link Worker

Salary: £28,436.62.
Location: 
Seaforth and Litherland PCN.
Hours: 
Full-time post at 35 hours per week.
Term: 
Fixed term: Expected up until March 2024.
Accountable to: 
Living Well Sefton.

The Senior Social Prescribing Link Worker (SSPLW) will be employed by Living Well Sefton and will work as part of one team, with 15 other Link Workers across 2 host organisations, providing a Social Prescribing service to GP practices, aligned to the Living Well Sefton model.

The SSPLW will work holistically to support individuals to take control of their health and wellbeing based on a ‘what matters to me’ approach by connecting people to a broad range of community groups and services. The SSPLW will support people to increase their resilience and reduce the impact of health inequalities by addressing the wider determinants of health such as debt, poor housing etc.  The Link Worker will also support existing VCF sector groups to be sustainable; identify gaps in services, and work with the Sefton CVS Group Development Team to develop new initiatives to meet local needs.

The SSPLW will have additional responsibilities within the PCN patch for the following:

  • Oversight of practical client management for the team
  • EMIS use, such as monitoring EMIS for unmatched documents, dealing with inappropriate or incomplete referrals
  • Monitoring non-referring surgeries and supporting communication and engagement
  • Direct supervision and monitoring of the PCN Social Prescribing Link Worker Team
  • Attendance at PCN and surgery meetings, representing the team where appropriate and keeping good lines of communication open with all stakeholders; feedback on emerging gaps and trends for reporting purposes within the area
  • Monthly feedback to administrator of Sefton CVS Here4You directory of new organisations in the area or changes to existing provision
  • Regular liaison with Living Well Sefton Social Prescribing Programme Management and support of key developments in the wider team.
