Salary: £23,316 for full time post.
Hours per week: 40 hours or 2 x 20 hours – Monday – Friday.
Reporting to: Operational Manager.

As part of Local Solutions, Liverpool Carers Centre delivers a city-wide community-based programme of activities and training that ensures a range of support is available to unpaid carers in Liverpool. This focuses on helping to reduce the negative impact of the caring role and promotes carers’ health and wellbeing, as well as undertaking carers assessments, their Carers Centre also offers a flexible support service that provides impartial advice & information.

They offer a range weekly and monthly activities and training throughout the year at the Carers Centre or at one of their community-based outreach hubs.
Job Description
- To work with GP practices across the city to raise awareness of carers known to them
- To set up a pathway to receive referrals from GP surgeries re carers
- To support carers to access the relevant support dependant on their need
- To give presentations, attend meetings and provide information to GP practice staff
- To ensure that carers identified by GP practices are supported
- To set up a process to support GP practices to identify carers
- To be part of the health and wellbeing team within the carers centre and take part in events and activities that will offer support to carers
- To use a variety of social media platforms to help raise awareness
- To provide monthly KPIs and reports as required
- To take on other roles that will support this piece of work
- Experience of working in health or social care
- Experience of supporting unpaid carers or working with people
- Experience of working as part of a team but the ability to prioritise own workload, managing own diary, building relationships with both carers and professionals, management databases, taking referrals from and inputting information
- Attendance at occasional evening or weekend
- Technical expertise should include ability to use Microsoft office, including spreadsheets
- The successful candidate will require an enhanced Disclosure & Barring Check and undertake mandatory safeguarding training as appropriate for the role
- Travel is required so a car owner/driver is required as this is a city-wide post working both in the office and in the community.
