NHS Cheshire & Merseyside is looking to develop a model of care designed to test a new way of working with vulnerable families in the city.
This proposal will help explore if there is a different way for the public sector to meet the needs of families facing significant distress, poverty, poor health, and broader life challenges.
A key principle being tested is that the third sector may be better placed to build effective and trusting relationships with families who experience many complex issues in their lives, and as such can sometimes have a poor relationship with, or experiences of, public sector services.
Initially, the focus is on the North Liverpool geography, though this may be expanded upon if the early stages are successful.
This delivery model will be suitable for third-sector organisations with experience working with ‘Complex Families’ and can help provide an effective bridge between them and health and care services (with the potential to broaden into wider public sector areas of need).
The aim is to facilitate more meaningful interactions between families and the services involved, with a view to improving outcomes and reducing the level of fragmentation and duplication that families can experience. The estimated value will be up to £400,000 over the period 2025/6-2026/7.
To develop this approach in partnership with local third sector organisations and other key stakeholders, NHS Cheshire & Merseyside are planning an engagement session on Thursday 6 February 2025 in Liverpool L5.
This session is for organisations who have experience working with these vulnerable families, located in North Liverpool. If the geographic spread or scope widens in the future, further engagement sessions will be held.
