Sefton CVS – Community Cancer Navigator

Salary: £26,484.31 per annum.
Hours: Full time, 35 hours per week.
Contract: Fixed Term to 31 March 2024.

This post will contribute to tackling inequalities in cancer care through the offer of support and to access a Holistic Needs Assessment (HNA) for people in south Sefton who are living with or affected by cancer. The Community Cancer Navigator will support anyone living with or affected by cancer who is registered with a GP practice in south Sefton to identify clinical and non-clinical needs and/or concerns and provide the right support to meet these needs in order to promote self-management and independence.

The post holder will work alongside the Primary Care Networks (PCNs), particularly with GP Practices to establish a robust referral pathway as well as with the Clinical Nurse Specialists (CNS) and Lead Nurse in the Cancer Community Support Service (CCSS) at Aintree University Hospital (LUFT).

The Community Cancer Navigator will be a practical resource to help connect individuals into services at a local level to aid independent self–management of their treatment and improve their health and wellbeing following 4 overarching principles of the service:

  • Everyone registered with a south Sefton GP practice who is living with or affected by cancer can have support from a Community Cancer Navigator.
  • Everyone registered with a south Sefton GP practice who is living with cancer will have the opportunity to have a conversation about their needs and concerns, and get the support that’s right for them at a time that’s right for them.
  • Everyone registered with a south Sefton GP practice who is living with cancer will have their vital needs met by high quality services.
  • Everyone registered with a south Sefton GP practice who is living with cancer that is treatable but not curable will be supported to live life as fully as they can.
