Reporting to: Sefton CVS Health Programmes Manager.
Location: South Sefton.

Sefton Council for Voluntary Service (CVS) is an independent registered charity with a borough-wide brief to assist and support voluntary community and faith sector (VCF) activity across Sefton, Merseyside.

Job Details

Social prescribing empowers people to take control of their health and wellbeing through referral, to non-medical ‘link workers’ who give time, focus on ‘what matters to me’ and take a holistic approach, connecting people to community groups and statutory services for practical and emotional support.

The role will work as part of a multi-disciplinary team focused on providing personalised care to predominantly; frail, older and vulnerable people, who want to live at home to do so, whilst maintaining a good quality of life.
