Salary: £26,484.31.
Location: Bootle, Crosby, Maghull PCN.
Hours: Full-time post at 35 hours per week.
Term: Fixed term: Expected up until March 2024.
Accountable to: Sefton CVS Line Manager.

Sefton CVS and Bootle, Crosby and Maghull Primary Care Network are recruiting two experienced and motivated individuals, to work with older people to help them to continue to live at home and experience enjoyable lives. By spending time with a person, Social Prescribing Link Workers help people to unpick what may be holding them back.  They support people to identify and connect with organisations and activities in their community.

This exciting opportunity will require the post holders to work as part of a multi-disciplinary team, supporting vulnerable and elderly people to continue to live in their own homes; using a holistic approach to assist independent living and reduce hospital admissions.

They are looking for team players who have strong communication skills, with the ability to build good working relationships with people from a wide range of social, cultural and ethnic backgrounds. The successful candidates must also be able to work independently and plan and organise their workloads.
