Sefton CVS – High Intensity Use of Services: Outreach and Intervention Worker

Contract: 12-month Fixed Term Contract in the first instance.
Term: Full-time (would consider part-time).
Salary: £26,484 per annum.
Hours per week: 35 hours.
Reporting to: Health Programmes Manager.
Location: Burlington House, Southport Community Centre and various locations across the Sefton borough.

Sefton Council for Voluntary Service (CVS) is an independent registered charity with a borough-wide brief to assist and support voluntary community and faith sector (VCF) activity across Sefton, Merseyside.

Job Summary

Sefton CVS and Cheshire & Merseyside ‘Health and Care Board’ are currently delivering a project to support high intensity use of emergency NHS services, known as ‘frequent attenders’. They are excited to announce that they are seeking an incredible and forward-thinking person to join their team, who can connect with people and provide 1:1 coaching to people who are going through difficult times.

The successful candidates will support people who feel they have nowhere else to turn but A&E or their GP surgery, more than expected. The aim of the programme is to provide a de-medicalised, human approach that doesn’t punish people for being in crisis. Evidence shows that if they concentrate on people and what they need, the results follow and healthcare contacts naturally fall as people get the time, understanding and solutions they need.

The role will require you to build strong relationships with the Sefton community, healthcare professionals, working closely with GPs, link workers and other multi-disciplinary teams to facilitate joint working to ensure that creative, sustainable and person-centred support is given.
