Sefton CVS – High Intensity Use Of Services – Outreach & Intervention Worker

Salary: £26,484 per annum.
Hours: Full time 35 hours per week.
Contract: Fixed term for 12 months initially.

Sefton CVS, South Sefton and Southport and Formby CCG’s are currently delivering a project to support high intensity use of emergency NHS services, known as ‘frequent attenders’.

They are excited to announce that this service is expanding and they are now seeking experienced, motivated individuals, with experience of home visits, lone working and supporting people with complex needs as well as working within a fast-paced environment, to join their rewarding service.

The successful candidates will work with a rolling cohort of patients and a varied caseload, who attend A&E and are admitted to hospital more than might be expected. They are looking for individuals whose excellent listening and communication skills, along with negotiating and leadership skills give them the ability to connect with patients, in order to change their behaviours. The post holders will seek to appropriately reduce the number of unscheduled care interactions, emergency contacts, hospital admissions and other care interactions across Sefton.

Reporting to the High Intensity Use of Services Co-ordinator, the Outreach and Intervention Worker will work with people to identify the real reasons for attendances, de-medicalise these reasons and focus on using a very human and 1:1 coaching approach to address those issues that are contributing to inappropriate care-seeking behaviours.

The ability to travel throughout the borough of Sefton in order to get the job done is important.
