Salary: £26,484 per annum.

The Hospital Discharge Team are looking to recruit 2 dynamic and forward-thinking people to expand the current service, which helps Sefton residents settle back home following a stay in hospital, and to prevent readmission. The successful applicant must be passionate about tackling health inequalities and have a person-centred approach that encourages these adults to set up a supportive network of family, friends, and neighbours, as well as their wider community.
This is a rewarding role for anyone wishing to make a difference in the health and well-being of adults. You will liaise with local hospital wards, as well as the hospital social work teams, to ensure referrals are appropriate and made in a timely manner to ensure a suitable discharge service can be provided.
The post will require you to conduct home visits as a lone worker. Experience in this field of work is desirable, along with excellent listening and communication skills. Your negotiation and leadership skills will enable you to deal confidently with clients. You may be required to liaise with a team of Volunteers and need to ensure the discharges are evenly distributed between them based on their availability and locality. You should be comfortable working both independently and as part of a team in order to manage this specialist area and a varied caseload. Good planning and organisational skills are essential together with the ability to adapt quickly to meet the needs of different clients and daily discharges.
The ability to travel throughout the borough of Sefton in order to get the job done is important.
