Hours: 21 hours aweek
Location: Sefton CVS, Burlington House, Waterloo, with work in primary care settings in Bootle
Job Purpose:
- Work supportively with people within primary care settings to improve their health and wellbeing in a sustainable way
- Work with individuals and groups in community settings promoting sustained behaviour change and health improvement
- Support the wider capacity building work of Living Well Sefton working in partnership with other community providers

Community Connectors Coordinator
Salary: £25, 440
Hours: 35 hours a weeek (full-time)
Contract: Funded for 18 months in the first instance
Location: Sefton CVS, Burlington House, Waterloo
Job Purpose:
- Co-ordinate and line-manage a team of Community Connectors operating in different localities across the Borough of Sefton
- Develop systems of recording and evaluation that demonstrate the effectiveness of the asset based approach in relieving pressure on the Health and Social Care system
- Support partners across local government and the NHS to build confident and connected communities as part of a strategic approach to reducing the experience of loneliness and isolation within our communities and to directly address problems of inequality
