The Whitechapel Centre – Hospital Discharge Worker (Support Worker)

Hours: 37.5 hours per week. Monday to Friday but with some flexibility to provide weekend cover by arrangement.
Salary: National Joint Council (NJC) SCP 11 – 22 (£21,748 – £27,041).
Location: Liverpool.

The Whitechapel Centre’s mission is to prevent and resolve homelessness, social exclusion and housing poverty by providing practical, realistic and tailored support. This is so that each person can find a route out of homelessness, maintain a home and achieve their individual potential. They provide targeted support to people who are homeless, at risk of homelessness or have a street lifestyle.

New funding has been secured which will enable The Whitechapel Centre to work in partnership with Liverpool City Council, Liverpool Clinical Commissioning Group, Brownlow Health, Liverpool YMCA, Mersey Care NHS Foundation Trust and The Royal Liverpool & Broadgreen University Hospitals NHS Trust. They will develop an integrated model of health care and support for people who present as homeless or at risk of homelessness in the Royal Liverpool & Broadgreen University Hospitals. This will ensure that they are supported into appropriate accommodation solutions and are safeguarded against the risk of rough sleeping on discharge.

The job holder will provide both in-reach and community-based housing-related support, working as part of a multi-disciplinary team. They will be acting as a liaison between the Hospitals and Local Authorities with regard to the discharge process under the provisions of the Homeless Reduction Act 2018 and other relevant legislation.

Ideally, the postholder will have experience of working in a hospital or social care setting and a wealth of experience delivering effective support services to people who are vulnerable or in housing need. You will have the passion and people skills necessary to work in partnership to develop appropriate solutions that meet the identified need, drive and commitment to see them through.
