Job Title: Community Wellbeing Officer
Salary: Salary: £19,500 (12-month fixed term contract with 6 month probationary period)
Hours: 37.5 hours per week

Purpose of post:
We’re looking for a passionate and empathic individual with qualifications and/or experience in community-led work to play a crucial role in engaging patients and communities in health and wellbeing activities. The successful candidate will be working collaboratively and creatively with local NHS Hospital Trusts, primary care, patients, clinicians and wider stakeholders to generate better health outcomes for patients.
Key responsibilities:
Undertake holistic one-to-one consultations with patients that address the social determinants of health
Deliver evidence-based interventions and training to patients, staff, and clinicians
Coordinate and deliver a varied range of educational and social inclusion courses to members of the public.
Work collaboratively with patients and clinicians to implement local action plans that address health needs and create opportunities for patients to improve their health and wellbeing.
Work closely with clinical teams around utilising community centred approaches to improve the health and wellbeing of patients
Enable and support individuals to change their behaviour to improve their health
Actively participate in clinical Multi-Disciplinary meetings, Patient Group meetings and attend wider networking opportunities; developing opportunities to work with others and supporting integrated working
Play a crucial role in the monitoring and attainment of key targets for Wellbeing Enterprises’ contracts, using a range of validated qualitative and quantitative methods
Recruit, induct, train and support volunteers
General administration duties, for example data-inputting, production of promotional materials and making/receiving phone calls

Job Title: Macmillan Community Wellbeing Administrator
Salary: £17,500 (6 month probationary period)

Responsible to: Macmillan Community Wellbeing Project Manager
Purpose of Post:
We’re looking for an experienced and highly organised Administration Officer to ensure the smooth running of community wellbeing services working in partnership with wider stakeholders.
The successful candidate will provide administrative support to the project delivery teams and will be responsible for developing and implementing efficient and effective administrative processes and procedures.
Key responsibilities:
To receive and validate referrals from a wide range of sources and contact the patient to arrange a convenient appointment
To provide comprehensive administrative support to the delivery staff
Book venues and prepare equipment and materials for wellbeing one to one appointments and group activities
To assist in the production of a range of marketing and promotional materials
To assist with the social media duties for the organisation including maintaining Twitter and Facebook
Develop and implement robust administrative processes and procedures to ensure the efficient delivery and monitoring of the service
Organise and coordinate meetings using online calendars and monitor uptake of group activities using online and telephone systems
Take minutes and prepare agendas and papers for internal and external meetings
Maintain paper and electronic records in line with the requirements of the Data Protection Act
General administration duties including: filing, photocopying, answering telephone calls, taking messages and data inputting
To coordinate the work of any administrative apprenticeships or assistants in the organisation
To carry out volunteer induction sessions and liaise with volunteers, patients, partners and the wider public
