WIRED Vacancy: Third Sector Connectors (2 posts)

Salary: ​£13,156 per annum (actual salary based on a £18,418.40 FTE)
Hours: 25 hours per week (Initially Mon/Fri, subject to adjustment dependent on service needs)

Wirral-based charity WIRED is part of a multi-organisational framework that delivers Early Intervention and Prevention Services to vulnerable adults and children across Wirral. Within the adult element, it is a requirement to support service users currently within primary care, secondary care and health and social care provision in Wirral to access community and voluntary based health and wellbeing provision within Wirral, in order to facilitate positive outcomes for the service users in relation to their health and well-being.

The role of the third sector connector will be to liaise with Health and Social Care professionals to identify, support and connect vulnerable adults to services and activities in the community that will encourage community support, independence and improved health and wellbeing. WIRED is recruiting two posts, based in Birkenhead.

The following skill areas are essential for this role:

  • Knowledge and understanding of Early Intervention and Prevention based services in Wirral
  • Robust knowledge of Wirral community and voluntary sector health and wellbeing sector providers and services available
  • Experience of case management within service provision
  • Experience and use of monitoring and feedback processes

Full driving licence and access to a vehicle for work related travel is essential.
